Evaluating Teachers (November 2005)

I am currently working at a new emerging yoga studio. The director would like to develop a way to evaluate teachers and the studio. Can anyone recommend a procedure or send me copies of evaluation forms that have worked well for you? Any suggestions would be appreciated.

Below are two types of form that I have used, in numerous variations, to elicit students' feedback on their experience of our yoga classes. One includes open-ended questions combined with a rating system. The other has only open-ended questions, with only minimal direction offered. Both forms have elicited rich and valuable comments, and are given out at the end of a series or of a longer workshop (3 hours or more).
