Registration Form

Each person should complete a form and send it in by e-mail, fax, or post. Have questions? Please don’t hesitate to contact us.

1. Download one of these versions of the application:

• PDF version, for “snail” mailing or faxing to us
  (Free Adobe Reader required)
• Microsoft Word version, for e-mailing to us
  (Microsoft Word or the free OpenOffice.org required)

2. Fill out the form and send it in.

For the PDF:

• Print the application, fill it out, and mail or fax it to the address or phone number on the form.

For the Microsoft Word version:

• Fill it out on your computer, save it to “My Documents,” and e-mail it as an attachment to info@expandinglight.org.
